2. Scan the barcode from the printed page. Output from
the scanner should display directly in this document. Repeat for all the barcodes listed. If you
have additional scanners, repeat the steps. If you encounter problems while scanning a
barcode, type "NO OUTPUT" into the document.
3. Once completed - please confirm at the bottom of this page.
Note: If you are not able to scan the barcode directly into this document, use Notepad, WordPad,
Simple Text, or another text editor to display the output. Write the exact output next to the
barcode.
